Table-Of-Contents generates invalid Index, all root indexes are counted multiple times

      When an issue is open, the "Fix Version/s" field conveys a target, not necessarily a commitment. When an issue is closed, the "Fix Version/s" field conveys the version that the issue was fixed in.

      Description

      See attached sample. The report produces an index of "4.1" for the first element, even though it should have generated it as 1.1. This is a regression on version 4.8.
